How do i get a nevada state cannabis card


The Nevada Department of Motor Vehicles (DMV) no longer processes medical marijuana cards in person. To request an online medical marijuana card application, visit the Nevada Department of Health and Human Services website at: https://dpbh.nv.gov/Reg/MM-Patient-Cardholder-Registry/dta/Forms/Medical_Marijuana_Patient_Cardholder_Registry_-_Forms/qualifying .
The qualifying debilitating medical conditions include: Acquired Immune Deficiency Syndrome (AIDS), Post-Traumatic Stress Disorder (PTSD), Cancer, Glaucoma, Cachexia, Sever pain, Sever nausea, Seizures, including without limitation, seizures caused by epilepsy, Persistent muscle spasms, including, but not limited to, spams caused by multiple sclerosis.
Any healthcare provider who is currently licensed to write a prescription for a chronic or debilitating medical condition and is in good standing with their Nevada Board can complete and sign the Attending Healthcare Provider Statement.
