An individual can act as a legal caregiver for up to five (5) registered patients. You must first be designated by a certified patient, and the certified patient’s registration must be approved by the New York Department of Health, before you can register as the certified patient’s designated caregiver.

Yes, you can! If you visit the NYDOH website at https://www.health.ny.gov/regulations/medical_marijuana/patients/ you can find out how to obtain your caregiver card. Patients can register up to two caregivers through the state.
Once certified, patients must login to https://my.ny.gov/, click the "Health Applications" icon, and then click the "Medical Marijuana Data Management System" link to register. Patients who do not have an NY.gov account will need to click the "Don’t have an Account?" button to create a personal NY.gov ID first.
Patients will be required to provide proof of their identity and residency during the application process. Click here for additional information regarding required documentation.
Pursuant to the Compassionate Care Act there is a $50 application fee; the Department is currently waiving the $50 fee for all patients and their designated caregivers. Simply select "Bill Me Later" when registering and the application fee will be waived.
The DOH website is really helpful. I encourage you to visit the site and learn how you can obtain your caregiver card.
