The only documentation we will require from you is a valid state or government issued photo ID. You are not required to upload any medical documents; just be sure to fill out the medical history section of the registration with as much detail as possible. Your doctor will have a chance to view this before they meet with you.
Upon approval, you can immediately print a PDF copy from your HelloMD account and you will also receive an embossed recommendation with your ID card in the mail.
