You just need to complete the medical intake forms (on our website) and have the video consultation with the doctor. If you have other medical information available, you can have it with you in the meeting with the doctor, or upload it prior, but this is typically not necessary.
You will also need to have a government issued ID, and be in the state of CA at the time of your consultation.
